Junior Varsity (5th and 6th Grade) Boys:

Please note that if the tryout begins at 5:30PM, players are to dismiss from school, and return to school at 5:30PM.

Medical Forms
All medical forms must be received before your child can participate in clinics, preseason practices, and regular season practices. Sudden Cardiac Awareness, Concussion Awareness, and Sports Physical Forms need to be submitted yearly and last for the full school year. You do not need to re-submit forms for each season.

Sports Fee Information
The sports fee will be charged each season of participation, varying by sport, and billed through Facts to supplement the Athletic Department Budget.
Uniform Ordering
All new team members will purchase his/her own uniform. Players will be responsible for care and maintenance, and the goal is for players to wear their uniform for the duration of participation at AAA. We have partnered with The Run Around in Roslyn for our uniform needs. Please note that the jacket is an optional item to purchase for all teams.

Transportation

Special Medical Needs
Please inform the Athletic and Health Offices if your child has any special medical conditions (such as allergies, asthma, cardiac conditions, diabetes, etc). It is the student’s responsibility to give to the coach any special medications that he or she carries so that it is available for use, if necessary, during games and practices both on and off campus.
