The Ancillae-Assumpta Academy Health Office is staffed by Fran Lantieri RN, Health Office Coordinator, Gina O’Malley RN, School Nurse,  and Sue Saloma RNC, School Nurse. 

In an effort to meet your child’s health needs at school, please notify the Health Office with all pertinent medical information and return all required Health forms by September.

For your convenience these forms will be mailed to you and also available to download. Please return the Health forms in a confidential envelope marked Nurse’s office. (you may want to make a copy for your personal file)

If your child has any special needs, please call,  e-mail, or fax  us so we can meet with you before September 2011 to coordinate with you a care plan for the school year.

During the school year, please notify the Health Office if your child contracts a communicable disease such as: chicken pox , Hand, Foot and Mouth Disease, Conjunctivitis (pinkeye), Fifth Disease, Head Lice , or Meningitis.

Any student who has a fever should remain home until a normal temperature is maintained for 24 hours without medication.

If a child arrives at school feeling ill or becomes ill during the day the school nurse will call you to pick up your child as soon as possible

It is required that both a physician’s note and the written permission of the parent/guardian be on file in order to take either prescription or non-prescription medicine in school.

Parents are urged to comply with the school’s Food Allergy Policy. No food items can be brought to school for any event without consulting the health office or food service coordinator. To ensure a healthy and safe environment for all students.
